About the Job
Primary Responsibilities:
- Provide skilled nursing care in accordance with the client’s physician orders, individualized plan of care, and applicable DSCC requirements.
- Assess the client’s physical, cognitive, behavioral, and medical needs and monitor for changes in condition.
- Administer medications and treatments as prescribed.
- Monitor vital signs, symptoms, and overall health status.
- Provide and/or oversee care related to medical equipment and prescribed treatments.
- Maintain accurate and timely nursing documentation, including assessments, medication administration, treatments, progress notes, and changes in condition.
- Recognize signs and symptoms of medical deterioration and take appropriate action, including contacting the physician, guardian/caregiver, or emergency services when indicated.
- Communicate effectively with the client, family/caregiver, physicians, therapists, case managers, and other members of the healthcare team.
- Follow infection-control, safety, emergency, and agency policies and procedures.
- Assist with activities of daily living and other supportive care within the scope of nursing practice and the established plan of care.
- Provide education and support to the client and caregivers regarding medications, treatments, health maintenance, and safety.
- Maintain client confidentiality and comply with HIPAA and all applicable state and federal regulations.
- Participate in care-plan reviews and communicate recommendations or concerns regarding the client’s ongoing needs.
- Promote the client’s dignity, autonomy, independence, and participation in community activities whenever safely possible.
